New Patients

If you are interested in becoming a patient in our practice, please print the following forms and bring them with you or just submit them online.

• Medical History Form
• Registration Form
• General Treatment Consent Form

We accept most insurance programs, as long as they will allow you as a patient to choose any Dentist to be your primary dental care provider without any obligation to see one of their Dentists within their network.

We are not managed care, therefore we do not accept any managed care plan such as HMO, PMI, DMO or any other capitation plan. We do provide dental care as primary care provider (PPO) based on contract with the following insurance companies (PPO plans only):

• Aetna /Prudential
• Cigna
• Delta Dental (Premier Plan)
• MetLife
• Principal PPO

We also work with Care Credit, Chase and the following to help finance your dental work. We also offer an In House Payment Plan.
